Children, Youth and Families
Wraparound
Wraparound is a strength-based
planning process that follows a series
of steps to help children and their
families realize their hopes and
dreams. Wraparound is a planning
process that brings people together
from different parts of the whole
family’s life, and is a team-based
activity that helps groups of people
involved work together toward a common goal. Wraparound is organized and
delivered by someone who is trained to facilitate the team. The team creates
a plan that includes ways to assure the children/youth and their families can
experience success in their communities, homes and schools.

Case Management and Supports Coordination
This service helps the family to obtain needed services that have been
identified. Services such as accessing, planning, advocating, coordinating and monitoring may be needed.
Parent and Youth Led Supports
Parent and youth-led supports connects individuals to supportive organizations and other people who have had similar life experiences. Parents and youth are the experts in understanding their life and support needs.

Respite Care
This service is intended to support the parent or primary care giver. It is
provided on an intermittent or short-term basis due to the absence or need for relief for the parent or unpaid care giver.
- Respite care is usually for a few hours or a full day
- In some cases, services may include overnight or full weekend stays with age appropriate peer groups
- Services may be where the provider picks up the child for an activity or at a specific location

Home BaseD
Intensive services for children and families, provided in their home and in the community. Services focus upon promoting normal child development, healthy family functioning, supporting and preserving families. Services are strength-based, emphasize parent involvement, parent and worker teamwork, and partnering with other service providers necessary.

Community Living Supports
Assistance, medical and behavioral training provided to children and their
parents in order to promote positive behavior and to improve overall health. The goal of this service is to maintain children in their natural home.

Summer Options
Activities for children and youth are offered through a variety of programs. Summer programs, such as summer daytime and overnight camps, are available. In order to assist families in taking advantage of summer opportunities, camp scholarships are available for children and youth.
